 You can get a replacement license online or by mail. You will need to pay $2 for the license. Only one copy of each license can be issued, and it will be the plastic credit card type. The date on your replacement license will be the date we issued the replacement. We cannot issue replacements for expired licenses.

Online:

1. Go to http://registry.faa.gov/amsvcs.asp

2. Create an account if you don't have one.

3. Log on and follow the instructions to get a replacement license.

Allow 1-2 weeks to receive your replacement in the mail.

By Mail:

1. Go to http://www.faa.gov/licenses_certificates/airmen_certification/media/8060-56.pdf for a form to request replacement of your lost or destroyed airmen certificate.

2. Mail the form or a signed, written request statingyour name, date and place of birth, social security number and/or certificate number, and the reason for replacement to:

Include a check or money order for $2 (U.S. Funds) for each license, made payable to the FAA. If your current address is listed as a Post Office Box (POB), General Delivery, Rural Route, or Star Route, please provide directions or a map for locating your residence.

Allow 4-6 weeks to receive your replacement in the mail.
